Welcome to the Shop (“Skin Care Essentials”) located on our website, https://www.EricWeissMD.com. This website and the Shop on this site are owned and operated by Eric Weiss, MD, FACS, ASPS (Plastic Surgery & Regenerative Medicine) in Orange Park, FL.

We understand that privacy online is very important, and we value your privacy. Therefore, we ask you to please read our Privacy Policy carefully before you proceed to order a product(s) from our Shop.

SATISFACTION GUARANTEED

We want you to be 100% satisfied with every Skin Care Essentials’ product(s) that you have purchased directly through our website’s Shop. If we do not meet your expectations, you may return any item(s) from your order within thirty (30) days of the original purchase date.

HOW TO RETURN

Please contact our office at (904) 215-5800 (Monday through Friday) during regular business hours to request a Return or Refund Authorization. This form will give you instructions on how to return a product. Returns are required to have a Return Authorization confirmed within thirty (30) days of the order date to be eligible to be processed for a refund. We do not provide pre-paid shipping labels. However, we do encourage you to use a shipping resource with a tracking system for any return.

  • A “BOX” ASKING IF THE PRODUCT WAS “DAMAGED IN TRANSIT” TO THE CUSTOMER
  • ask the customer to attach a photograph(s) of the box and product(s). Be sure the office has designated as “Shop Customer Service Staff” to take an issue call.

DAMAGED PRODUCT

We apologize if you have received a product(s) that may have been damaged during transit. First, request the Return Authorization form; photograph the box and any damaged product(s) upon receipt; attach the Return Authorization form and photograph(s); then email both to us. We ask that you please save the damaged box and/or product(s) to send back. Please call our office at (904) 215-5800 within two (2) days of receipt of the damaged product(s) for further instructions.

LOST PRODUCTS

We are not responsible for any lost or stolen product.

RETURN OPTIONS

You can return your product(s) for store credit, a different product, or a refund to the original payment method.

RETURN SHIPPING

Refunds will exclude all S&H (shipping and handling) charges as well as any costs incurred for return shipping. Again, any products returned after the thirty-day (30) period are NOT eligible for a refund. We will not accept any damaged products returned to us without pre-approval on the Return Authorization. Shipping insurance is your responsibility.

ACCEPTED PRODUCT RETURNS

We are only able to accept products for refund that have been originally purchased through our website’s Shop. We reserve the right to refuse a return.

CREDIT CARD REFUND

If you paid by credit card (debit or credit), refunds from our Shop will ONLY be made to the original card used from which the product(s) was purchased from our Shop.
