There are many reasons why women may choose to undergo breast explant surgery. Reasons may include discomfort, capsular contracture, rupture, or maybe you just simply changed your mind. Some women want their implants taken out because they believe that the implants are making them sick. Whatever the reason, Dr. Weiss will support your decision and walk you through the options.

Procedure

A capsule will have formed around your implants. A capsule is a special type of corrective tissue that forms around all types of medical devices including pacemakers, artificial joints, chemotherapy ports, etc. It is a normal reaction of the body. 

When having your breast implants removed, you may or may not want this capsule removed as well. Dr. Weiss will discuss the pros and cons of a capsulectomy. When having an explantation for breast implant illness, the capsule is always removed. There seems to be some misinformation around capsulectomy and explantation for BII. This centers around the capsule and breast implant being removed en bloc – or in “one piece.” If your implants are placed under the pectoralis major muscle, the en bloc method of capsulectomy is often impossible without causing injury to other structures. Dr. Weiss will always attempt the en bloc technique first – but will ultimately remove the capsule the safest way possible. Rest assured; the capsule will be removed in its entirety.  

The largest study to date involving 44 women with symptoms potentially related to their breast implants showed that symptoms were just as likely to improve no matter how the capsule was removed. The key is to remove the capsule in its entirety the safest way possible.  

Drains may or may not be necessary following the explant surgery. Dr. Weiss will make a decision in the operating room if drains will be beneficial to your healing. 

We can often use your existing scars to perform the explant, and scar management plays a large role in Dr. Weiss’ post operative care plan. 

Many patients add a breast lift or fat grafting at the same time as the explant procedure. This can help eliminate or minimize sagging or deflation of the breasts after removing the implants. These options can be discussed with Dr. Weiss during your consultation, and he will give his professional opinion.
